Sega Flash Vol 5

The disc contains a video of Resident Evil. The Sega Flash demo series was distributed in Europe through various magazines, or included with Sega Saturn consoles (mostly volumes 1 and 2). They were usually packed in a cardboard or plastic envelope, and sometimes in nothing. Sometimes magazines put them in a jewelcase and an ad was used as a front insert. The packaging variations for Vol 5 have not been documented, if there are any.
