The book "Heavier Than Heaven" by Charles R. Cross (page 187, paperback edition) mentions a Smells Like Teen Spirit 7" test pressing which they brought to a radio station when the band visited KXLU studios in Los Angeles on August 15 1991. It was their radio promotion guy who apparently had the record.
It does not mention any more details than this, but the only 7" made in the USA is DGCS7-19050 (see sliver.it, various variations exist). The date makes sense, the first Smells Like Teen Spirit singles were released September 10 1991, and test pressings are often made a good bit in advance of the retail pressing, or at least a while before the item is released.
