Notes

The second Japanese release featured Dual Shock compatibility and came with a second disc, Complete Disc. A manual, an obi, an advertisement card, and a survey/ registration card were also included.

Complete Disc contains the intro video and all endings to Bio Hazard, videos of Biohazard 1.5, and memory card saves for Bio Hazard, Bio Hazard Director's Cut, Biohazard 2, and Biohazard 2 (Dual Shock Ver.).
