The disc title is OPS2M Demo 12. It is the same disc as distributed in the UK, but it is not the same disc as the French nor the German demos with the same title.

It contains a Silent Hill 2 trailer. The German demo contains the same Silent Hill 2 trailer, but it is missing the Time Crisis 2 trailer, which was probably censored for being a light gun game. The French demo contains a different Silent Hill 2 trailer.

The back of the sleeve is plain white.
